All claims against the City are processed only after the City's claim form has been completed in full and submitted. If you wish to file a claim against the City, complete the Liability Claim Form below. Complete the form to the best of your ability; attach any estimates, invoices and photos you have pertaining to the claim.
The City will complete all paperwork necessary and forward your claim to the City's self-funded liability provider coverage, Montana Municipal Interlocal Authority ("MMIA"), for processing. MMIA will contact you shortly after receiving your claim.
