All claims against the City are processed only after a claim form has been completed in full and submitted. If you wish to file a claim against the City, download the Liability Claim Form. You have several options to file a claim: complete it on line and submit directly to us or print the form and complete it. Complete the form to the best of your ability; attach any estimates, invoices and photos you have pertaining to the claim. You can then submit it to us by mailing it to the address on the form, faxing it to 447-8434 or delivering it to Room 320 of the City/County Building at 316 N. Park Ave. The City will complete all paperwork necessary and forward your claim to MMIA for processing. MMIA will contact you shortly after receiving your claim.
